ALL MEDICAL AND IMMUNIZATION RECORDS ARE UNATTAINABLE DURING THE SUMMER SESSION.
FOR IMMUNIZATIONS: LAB WORK MAY BE PERFORMED BY YOUR HEALTH CARE PROVIDER TO PROVIDE IMMUNITY VERIFICATION FOR SEVERAL VACCINATIONS.
WE WILL BE HAPPY TO PROVIDE COPIES OF ALL IMMUNIZATION RECORDS WHEN WE REOPEN AUGUST 18, 2010
